I, the under signed parent/guardian of the above name of child do hereby consent to his/her participation in VBS at the El Centro New Generation Church (ECNG). I agee to waive and release ECNG and it's volunteers from all claims for damages that may arise, as a result of my child's participation in VBS. I am aware that the above name child may appear in a photograph, or video, taken by VBS staff, and that photograph or video may be used. I acknowledge and confirm that I have read this entire document before hitting confirmed and summiting this form.

Shirts ordered through this system will be given the week before the event or the day of the event. Please make sure to remind the front desk when at the event. For parents who would like to purchase a shirt please fill out the Volunteers/Parents seccion.
