I, the under signed parent/guardian of the above name of child do hereby consent to his/her participation in VBS at the El Centro New Generation Church (ECNG). I agee to waive and release ECNG and it's volunteers from all claims for damages that may arise, as a result of my child's participation in VBS. I am aware that the above name child may appear in a photograph, or video, taken by VBS staff, and that photograph or video may be used. I acknowledge and confirm that I have read this entire document before hitting confirmed and summiting this form.

Shirts purchase through this system will be given the week before the event or the day of the event. Please make sure to remind the front desk when at the event. T-shirt size are first come first serve.

If paying online their will be a small percentage fee already included on the price to cover for taxes, than if paying with cash in person. For more information speak with Pastora Monica to pay in cash.
